I acknowledge that pickup times may vary depending on route scheduling and operational requirements. A representative will contact me with the estimated pickup time and booking confirmation.

I understand that cancelling a scheduled pickup on the same day will incur a $20 call-out fee. To avoid this charge, cancellations must be made at least 24 hours before the scheduled pickup.

I understand that this booking form is not a Customs Declaration Form. I am responsible for completing and submitting the required Customs Declaration Form, either digitally or in paper format, during the Balikbayan Box pickup process.

I acknowledge that Eagle Cargo reserves the right to decline loading or collecting my box if a completed Customs Declaration Form and full payment are not provided at the time of pickup.
